Informed Consent
The informed consent form provides important information about our data collection activities and your rights as a participant in the evaluation process. We have prepared a single informed consent form that covers all of the evaluation questionnaires we plan to administer. You only need to provide informed consent one time. You may review the form whenever you like.

Before you respond to any evaluation surveys, please review the informed consent form accessed through the following link.

On-Line Surveys
We are asking directors and other staff involved in designing and delivering sponsored professional development projects to respond to two on-line surveys. Each survey will require you to enter a password before it will be displayed. We will provide passwords for the surveys to the director of your professional development project.

If you've reviewed and agreed to the informed consent, please complete the relevant surveys at the appropriate time using the links provided below.
