Informed Consent
The informed consent form provides important information about our data collection activities and your rights as a participant in the evaluation process. We have prepared a single informed consent form that covers all of the evaluation questionnaires we plan to administer. You only need to provide informed consent one time.

Before you respond to any evaluation surveys, please review the informed consent form accessed through the following link.

Participant Surveys
We are asking participants in sponsored professional development projects to respond to surveys at three different times. The surveys are organized based on when they should be completed. Simply click on a survey listed under the corresponding time period to open the survey in a new window.

Each survey will require you to enter a password before it will be displayed. We will provide passwords for the surveys to the director of your professional development project.

Near the end of the PD project or 2005/2006 school year

Once you have reviewed and agreed to the informed consent (see above), please complete the Teaching Philosophy Survey and the Teacher Participant Survey using the links provided below.

If you have not already completed a paper copy of the Principles of Practice then we ask that you please print a copy from the link below, complete it, and mail it to the evaluation team.

Examples of Completed Logs:

Spreadsheet for Summarizing Student Assessment Scores
(Pre and Post tests).Teachers can use this spreadsheet to record pre- and post-test scores from assessments taken by their students. Generally, these tests are part of units or lessons that the teachers developed as part of their participation in a PD project.
More detailed directions on how to complete the worksheet can be found on the “Introduction” tab (page 1) of the spreadsheet (file updated 11/08/2005).
